A verified severe incident requires cited evidence and material harm; a material capability change requires documented new or substantially improved behavior with plausible risk relevance; a governance response requires an enacted or formally announced safeguard. Missing evidence is shown as data unavailable, never estimated. Generation times are UTC; report dates use the configured local reporting date.
